A. All Penn Fusion Soccer Academy Coaches/Trainers must have a Risk Managemen/Background check on file. 

To complete your Risk Managment/Background Check please click on both of the following links and complete both forms.

 Click here to access the US Youth Soccer (EPYSA) Risk Management Form.

 Click here to access the US Club Risk Management Form.  Choose "Club" when prompted and then use the drop down menu for Penn Fusion Soccer Academy (#6949)

B. In addition, all Penn Fusion Coaches/Trainers must:

1. Register with the Penn Fusion Soccer Academy by clicking on the 2012-2013 Penn Fusion Coach Registration link and signing in using your gotsoccer coach account.

Please upload a recent photo into your gotsoccer coach account as this will be the photo used for your coach’s pass. 

Please check to make sure that your account information is accurate.

2. Complete the CDC Online Course titled "Heads Up Online Training Course" by clicking this link.

3. Print out the certificate at the end of the online course and send to the Penn Fusion Administrator.

C. Coaches must also complete a W9 Form:

1. Complete and sign a W9 each year.  Form can be found in left sidebar.  W9 to be sent to the Penn Fusion Administrator.
